The provider will be responsible for caring for patients, maintaining accurate and current patient records and scheduling and administering follow-up appointments to patients as required. The successful candidate will work as a team with our staff and other providers and will assist in delivering quality care to each and every patient. Duties and Responsibilities Perform initial patient screenings, history and physical examinations, and follow up visits for acute and chronic care management Update patient records and check records for accuracy at each patient appointment Maintain timely documentation of patient encounters with note completion within 48 hours of the encounter Analyze test data and determine the need for follow-up appointments and further treatment options Deliver quality care while maintaining all company metrics pertaining to productivity Serve as the resource patients need for ongoing care information, counseling and provide guidance for any patients with critical conditions Practice within scope of practice for the providers licensure Refer to outside specialists as appropriate, ensure timeliness of referrals Provide a timely response to refill requests Work closely with the Office Manager to ensure smooth patient flow Qualification and Skills Hold at least a Master's degree in NP.
