• 60/hour, depending on experience

Shift:

Mon-Fri 8a-5p + weekend rotation

Qualifications:

Required:

• Active Oregon Registered Nurse license
• Associate's or Bachelor's degree in Nursing
• Minimum of 1 year of recent acute care nursing experience
• Current BLS and hands-on CPR certification
• Valid driver's license with reliable, insured transportation

Preferred:

• Previous home health nursing experience
• Experience managing an assigned patient caseload
• Strong assessment, care coordination, and patient education skills

Job Summary:

• Complete comprehensive in-home nursing assessments and identify each patient's clinical and ongoing care needs.
• Develop, implement, and update individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team.
• Provide skilled nursing care, education, and support to patients and caregivers in the home setting.
• Monitor changes in patient condition and communicate updates or concerns to the appropriate members of the care team.
• Coordinate services and assist with discharge planning to support safe transitions throughout the patient's episode of care.
• Maintain accurate and timely clinical documentation while independently managing an assigned territory and caseload.
