Key Responsibilities:

Conduct comprehensive home health risk assessments, averaging 45 minutes per visit Evaluate patients prior and current medical history, ongoing conditions, and medication usage Educate patients on their health conditions and medications, supporting quality self-care Identify gaps in healthcare and offer actionable recommendations for improvement Focus solely on assessmentno prescribing or treatment planning is involved

Qualifications:

Active Nurse Practitioner license in Oregon Current CPR/BLS certification Board Certification through AANP or ANCC At least 6 months of recent Nurse Practitioner experience preferred Master of Science in Nursing (MSN) from an accredited program This per diem position offers competitive compensation, with $80$90 provided per evaluation, in addition to a mileage stipend and coverage for administrative time.
