Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members in Prineville, Ochoco West, and additional locations within Crook County. Review and evaluate patient medical histories, prior and current conditions, and medication usage. Deliver patient education on diagnosed conditions and prescribed medications. Identify care gaps, recommending strategies to improve patient outcomes. Each assessment lasts approximately 45 minutes and may include annual wellness assessments or post-discharge evaluations. No requirement for creating treatment plans or prescribing medications.

Qualifications:
