Key Responsibilities:

Perform annual home health risk assessments, typically lasting around 45 minutes each Assess patients medical histories, current health status, and medication regimens Deliver targeted education regarding medical conditions and safe, effective medication usage Identify gaps in care and provide clear recommendations to promote continuity of care Focus exclusively on assessment and educationno need to develop treatment plans or prescribe medications

Qualifications:

Current Nurse Practitioner license in Oregon Active CPR certification (BLS required) Board Certification through AANP or ANCC Minimum of six months' recent experience as a Nurse Practitioner is preferred Master of Science in Nursing (MSN) from an accredited institution You will serve members throughout Lane County, including Springfield, Eugene, Elmira, and Cottage Grove. Daily assignments may take you to any of these locations, offering variety and the chance to impact a broad range of individuals. Compensation for this position includes $80$90 per evaluation, plus mileage stipend and administrative time, reflecting your advanced skills and commitment to patient care.
