Key Responsibilities:

Complete comprehensive annual home health risk assessments for Medicare Advantage health plan members Thoroughly evaluate patients medical histories, current conditions, and medication regimens Offer evidence-based education and guidance regarding chronic and acute health issues, as well as proper medication usage Identify care gaps, offering actionable recommendations that promote continuity and quality of care Focus exclusively on assessment, counseling, and educationno requirements for developing treatment plans or prescribing medications

Qualifications:

Active and unencumbered Nurse Practitioner License in Oregon Current CPR certification (BLS) Board Certification from AANP or ANCC 6 months or more of recent Nurse Practitioner experience (preferred) MSN degree from an accredited nursing program This opportunity offers compensation of $80-$90 for each completed evaluation, plus a stipend for mileage and administrative time.
