Key Responsibilities:

Conduct comprehensive annual risk assessments and post-discharge evaluations in patient homes Review medical history, ongoing conditions, and current medication regimens Educate patients regarding their health conditions and prescribed medications Detect gaps in care and offer targeted recommendations to support improved wellness Focus exclusively on assessmentno treatment planning or prescribing required

Qualifications:
