This role works closely with an interdisciplinary team to develop and implement individualized care plans. Responsibilities Perform patient assessments and develop care plans Provide skilled nursing care in patients' homes Coordinate care with physicians and interdisciplinary team members Educate patients and families on care and disease progression Maintain accurate and timely documentation Ensure compliance with hospice regulations and standards Qualifications Current Registered Nurse (RN) license (Pennsylvania or Compact License) Hospice or home care experience preferred Strong communication and organizational skills Valid driver's license and reliable transportation #ocean2024
