Make a difference delivering compassionate care with this full-time, 13-week contract opportunity for an experienced Home Health Registered Nurse. Work Monday through Friday, 8:00 AM to 5:00 PM, in a field-based role visiting patients in their homes and providing high-quality nursing services throughout the assigned territory.

PM Key Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized patient care plans Deliver skilled nursing interventions including wound care, IV therapy, medication administration, and chronic disease management Monitor patient progress, document changes in condition, and communicate promptly with care teams and providers Educate patients and families on treatment plans, disease management, and preventive care Maintain accurate and timely records using Home Care Home Base (HCHB) electronic documentation Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated care delivery Attend case conferences, staff meetings, and participate in ongoing professional development Ensure compliance with regulatory requirements and agency care standards

Qualifications:

Active RN license in good standing Minimum of one year of recent home health (field) experience Proficiency with Home Care Home Base (HCHB) Experience completing OASIS assessments Reliable transportation, valid drivers license, and insurance
