Deliver essential nursing care directly to patients in their homes with this full-time, 13-week contract position. Working Monday through Friday, 8:00 AM 5:00 PM, you will manage a caseload averaging 30 points per weektypically translating to 35 patient visits daily. The field-based setting means youll have daily travel within your assigned territory, with mileage reimbursement provided for your convenience.

Key Responsibilities:

Conduct comprehensive OASIS assessments and develop individualized plans of care Deliver skilled nursing services including wound care, medication administration, IV therapy, and management of chronic diseases Monitor patient status, promptly identify and report changes, and communicate effectively with care teams and physicians Provide thorough education to patients and families about diagnoses, treatment protocols, medications, and preventive health Maintain current and accurate documentation using Home Care Home Base (HCHB) Collaborate closely with multidisciplinary partners including PT/OT, medical social workers, and home health aides Attend case conferences, participate in staff meetings, and pursue ongoing professional development Uphold regulatory and agency standards for quality, compliance, and patient-focused care

Qualifications:
