•As an Infection Preventionist Registered Nurse (RN) , you will have the ability to provide high quality care for residence here at Concordia. Qualifications of an Infection Preventionist (RN): Dedication to providing high quality care Current licensure as an Registered Nurse Excellent communication and interpersonal skills 3-5 years of nursing experience Excellent analytical and writing skills Basic computer skills Good understanding of quality management Good understanding of education principles The Infection Preventionist specialist training in prevention and control

Essential Duties and Responsibilities:

Conduct an educational needs assessment for skilled nursing based on input from the nursing staff, direct care observation, and other health care provider feedback. Utilizes results to develop and enhance nursing onboarding and continuing education. Coordinates and oversees an infection control program which includes investigating, controlling, and preventing infections in the facility, developing infection control policies and procedures and keeping them current, and maintaining records of incidents and corrective actions relating to infections. Participates in Antibiotic stewardship team, educating, tracking, and reporting. Completes all NHSN reporting. Completes weekly surveillance for infection and antibiotic use for mandatory infection reporting and submits this to DON to validate criteria for reporting and then follows with subsequent documentation. Assists in annual review/updating of nursing polices and procedures. Keeps current on best nursing practices, standards, and education. Identifies policies, standards, and practices not in compliance with current standards and communicate needed revisions to the Director of Nursing and Medical Director. Works with other providers in the continuum, including Personal Care, Homecare, and local hospitals to develop care pathways to ensure seamless transitions of care and excellent outcomes. Researches, develops, and implements care pathways to help guide staff in the assessment and management of common changes in resident status that result in acute care transfers. Provides evidence-based and expert-recommended assessment and management of common conditions. Promotes evidence-based educational methods and technologies that support development of nursing expertise. Assures that nursing staff meet required competencies, both annual and unit based, for area of practice. Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.
