Key Responsibilities:

Complete comprehensive home health risk assessments for Medicare Advantage plan members Evaluate and document prior/current medical history, active medical conditions, and medication usage Provide patient education regarding health management, medications, and chronic conditions Identify healthcare gaps and offer evidence-based recommendations to improve outcomes Focus exclusively on assessmentno treatment planning or prescribing is required

Qualifications:

Active Nurse Practitioner license in Pennsylvania Board certification through AANP or ANCC Current CPR (BLS) certification MSN from an accredited nursing school At least 6 months of current NP experience preferred You will complete each assessment in approximately 45 minutes, with visit types spanning annual health assessments and post-discharge evaluations. Scheduling is location-dependent; visits may occur in Erie, Millcreek, Corry, and other parts of Erie County. This per diem opportunity provides competitive earnings, with $80$90 per evaluation plus mileage reimbursement and compensation for administrative time.
