Key Responsibilities:

Perform annual home health risk assessments for Medicare Advantage plan members Review prior and current medical history, medical conditions, and medication use Educate patients on their medical conditions, management plans, and medication regimens Identify care gaps and make recommendations to resolve identified issues No treatment planning or prescribing required

Qualifications:

Active Nurse Practitioner license in Pennsylvania MSN degree from an accredited nursing program Board Certification through AANP or ANCC Current CPR (BLS) certification At least 6 months of recent NP experience preferred Enjoy highly competitive earningseach risk assessment is compensated at $80-$90, in addition to a mileage stipend and pay for administrative time.
