Key Responsibilities:

Perform comprehensive home health risk assessments for Medicare Advantage members, typically lasting about 45 minutes Travel to patient residences throughout Scranton, Carbondale, Dunmore, and surrounding areas within Lackawanna County Review patient histories, document current medical conditions, and update medication records Educate members on medical conditions, therapeutic options, and medication management Identify care gaps and recommend solutions to optimize health outcomes No requirements for creating treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner License in Pennsylvania Current CPR (BLS) certification Board certification through AANP or

ANCC MSN

from an accredited nursing program Minimum six months current experience as a Nurse Practitioner preferred
