Job Summary Neighborhood House has a longstanding commitment to address social determinants of health and decrease health disparities in King County. The primary role of this position is to provide clinical leadership, medical expertise, training, and consultation to staff and clients in the Education & Community Services (ECS) Department at Neighborhood House. The Community Health Nurse reports to the Community Resources Manager. The programs in the ECS Department assist a large and diverse community-dwelling client population with multiple risk factors including housing instability, physical and/or behavioral health problems, long-term care needs, substance use disorders cognitive status concerns, and physical or emotional neglect or abuse. The Community Health Nurse will be a dynamic leader in our work to decrease health disparities, working closely with our team of case managers to address the health barriers facing our clients. The ideal candidate will have experience leading community-based health initiatives, navigating within a dynamic and flexible atmosphere, and working with high-acuity populations. This position will be primarily located in Seattle, Tukwila, and South King County, with hybrid remote/in-person work schedule of 2-3 days per week onsite at various Neighborhood House locations and client homes Responsibilities Meet with clients and families as requested; review client information, and provide input to client assessments, care plans, and service records. Provide nursing consultations to staff as requested; offer clinical insight to improve health outcomes; determine required nursing intervention/activities that may include making telephone calls to clients and/or home visits, follow-up to other health care professionals such as client Primary Care Providers and Home Health Care staff. Plan and perform onsite education and training on disease management and health promotion with staff, clients, and client caregivers. Implement health promotion activities in the community in the form of evidence-based workshops and education sessions. Lead educational and training sessions for case management, teacher, resource navigator, and non-clinical staff in various health and wellness topics. Actively develop and maintain relationships with community partners to assess and address gaps in health literacy and health promotion. Follow state and program policies and procedures for nursing referrals for a variety of risk factors including pain management and skin breakdown. Discuss medical/nursing issues and responsibilities with the department leadership on a regular basis. Lead monthly case reviews of medically complex clients with case managers. Provide referrals to needed services such as Adult Day Health program, Substance Use Disorder program, medication management, and other support services. Perform other duties as assigned.
