Nurse Practitioner - At-Home Physicians Services Grace Caregivers | Lexington, SC Grace Caregivers is expanding our services to include at-home physician services , and we are seeking a compassionate, experienced Nurse Practitioner (NP) to join our growing team. This is a unique opportunity for a Nurse Practitioner who enjoys providing personalized care outside of the traditional clinical setting. Rather than working from a physical medical office, our providers deliver care directly in the patient's home and through telehealth . The Nurse Practitioner will be responsible for conducting in-home evaluations, telehealth visits, ongoing patient assessments, treatment planning, care coordination, and follow-up care . Laboratory testing and other services will be coordinated through appropriate third-party providers. Responsibilities Conduct comprehensive in-home patient evaluations Conduct telehealth visits with patients when appropriate Assess patients' health concerns, symptoms, needs, and overall well-being Develop individualized treatment and care plans Provide ongoing follow-up and monitor patient progress Order and coordinate appropriate laboratory testing through third-party laboratory providers Review and interpret laboratory and diagnostic results Coordinate care with patients' existing healthcare providers, specialists, caregivers, and family members as appropriate Identify changes in a patient's condition and determine appropriate next steps Provide education to patients and families regarding health conditions, treatment plans, preventative care, and wellness Document all patient encounters, assessments, treatment plans, and follow-up appropriately Maintain accurate and timely electronic medical records Utilize telehealth technology to provide accessible care between in-home visits Help reduce unnecessary trips to urgent care, emergency departments, and traditional medical offices when appropriate Communicate with the Grace Caregivers team regarding patient needs and care coordination Maintain appropriate professional boundaries between medical services and Grace Caregivers' non-medical home care services Follow all applicable federal and South Carolina laws, regulations, professional standards, and organizational policies Care Model Our providers do not work from a traditional brick-and-mortar medical office .

In-home medical evaluations Telehealth appointments Third-party laboratory services Ongoing care planning and follow-up Coordination with patients, families, caregivers, and other healthcare professionals The goal is to make quality healthcare more convenient, accessible, and personalized by bringing care directly to the patient. Qualifications Current, unrestricted Nurse Practitioner license eligible to practice in South Carolina Current national board certification as an NP (e.g., FNP, AGNP, or other applicable certification) Prescriptive authority in accordance with South Carolina requirements DEA registration, if required for the scope of services provided Current CPR/BLS certification Previous experience in primary care, family practice, geriatrics, home-based care, urgent care, or a similar setting preferred Strong clinical assessment and decision-making skills Excellent communication and interpersonal skills Comfortable providing care independently in patients' homes Comfortable utilizing telehealth platforms and electronic medical records Strong documentation and organizational skills Valid driver's license and reliable transportation Willingness to travel locally for scheduled in-home visits Ideal Candidate We are looking for an NP who is: Compassionate and patient-centered Comfortable working independently Confident performing assessments outside of a traditional clinical environment Comfortable treating older adults and individuals with multiple healthcare needs Tech-savvy and comfortable with telehealth Organized and thorough with documentation An excellent communicator Interested in building a new, innovative model of healthcare delivery Passionate about making healthcare more accessible to patients in their own homes Schedule [Full-Time] The schedule will consist of a combination of telehealth appointments and scheduled in-home visits .
