We are looking for a Licensed Practical Nurse to join our leadership team in a part-time supervisory capacity and help us deliver on that promise. About the Role This is a field-based, part-time supervisory nursing role — not a bedside or skilled care position. As our LPN Supervisor, you will provide clinical oversight and guidance across our caregiver team, conduct in-home client assessments, develop and review individualized care plans, and serve as the clinical voice within our operations. You will work closely with agency leadership, caregivers, and client families to ensure care is delivered safely, consistently, and with the quality our clients deserve. What You Will Do Conduct in-home client assessments and document findings to inform individualized care plans Develop, review, and update care plans in accordance with client needs and agency standards Provide clinical orientation and ongoing training to caregivers on care plan implementation, safety protocols, and client-specific needs Conduct periodic supervisory visits to observe caregiver performance and ensure plan adherence Serve as a clinical resource for caregivers and office staff — answering questions, coaching in the moment, and escalating appropriately Collaborate with agency leadership and scheduling on client placement and caregiver-client matching Maintain accurate, timely documentation in compliance with agency and regulatory requirements Communicate with client families and referral partners as needed to support continuity of care Participate in quality assurance reviews and contribute to ongoing agency improvement initiatives Who You Are Active, unrestricted LPN license in the Commonwealth of Massachusetts — required Minimum 2 years of clinical experience; home care, community health, or long-term care experience strongly preferred Strong assessment skills and the ability to translate clinical observations into clear, actionable care plans Comfortable working independently in the field with minimal direct supervision Warm communicator — with clients and families, caregivers, and leadership alike Organized and detail-oriented; documentation accuracy matters in this role Reliable transportation and valid driver's license required — this is a field-based position serving clients across MetroWest communities CPR/First Aid certification required or willingness to obtain within 60 days of hire Familiarity with SmartCare or similar home care platforms is a plus Compensation & Benefits $35.00 - $42.00/hr based on experience Flexible, part-time schedule Monthly gas stipend Home Matters Caregiving is an equal opportunity employer committed to building a team that reflects the communities we serve.
