Summary We are seeking skilled nurses to join our team in delivering exceptional nursing care to clients in their homes. This role is crucial in supporting our mission of providing high-quality, personalized care that enhances the quality of life for our clients. Responsibilities Provide comprehensive nursing care to clients in their homes. Conduct thorough patient assessments and develop individualized care plans. Administer medications and treatments as prescribed. Monitor vital signs and report any changes in patient condition. Collaborate with family members and healthcare professionals to ensure continuity of care. Educate clients and families on health management and wellness strategies. Maintain accurate medical records and documentation. Ensure compliance with all health regulations and standards. Qualifications Valid nursing RN or LPN license in the State of Tennessee Current valid driver's license Strong assessment, clinical, and communication skills. Ability to work independently and manage time effectively. Experience with IV infusion and medication administration preferred. Knowledge of geriatric care practices is a plus. CPR certification Familiarity with electronic medical records (EMR) systems.
