Position Summary:

Seeking a compassionate Registered Nurse Case Manager to provide comprehensive hospice care to patients in their homes and coordinate individualized plans of care with patients, families, physicians, and interdisciplinary teams.

Key Responsibilities:

Conduct comprehensive patient assessments and develop care plans. Provide skilled nursing care, symptom management, and medication administration. Educate and support patients, families, and caregivers. Coordinate care with physicians and interdisciplinary teams. Monitor patient progress and update care plans as needed. Maintain accurate EHR documentation and regulatory compliance.

Requirements:

Active, unrestricted RN license . Minimum 1 year of hospice/RN experience required. Hospice or home health experience preferred. Strong assessment, communication, and care-coordination skills. Valid driver's license and reliable transportation. BLS/CPR certification preferred. CA02

Pay:

$80,000.00 - $90,000.00 per year
