One Solution Homecare is seeking an experienced, dependable, and self-directed Registered Nurse (RN) to join our clinical team on a part-time basis. This position is ideal for an RN who enjoys working independently, developing relationships with clients and families, and supporting high-quality care for individuals who wish to remain safely in their homes. The RN will provide clinical oversight of our non-skilled home care services, with a strong focus on client assessments, care planning, CNA supervision, and ongoing monitoring of client needs.

KEY RESPONSIBILITIES

Conduct initial client intake assessments and assist with establishing appropriate services. Complete CNA supervisory visits in accordance with agency policies and regulatory requirements. Perform scheduled client reassessments every two months. Conduct additional reassessments following a change in condition or status. Develop, review, and update individualized client care plans. Identify changes in client needs and communicate recommendations to clients, families, caregivers, and agency leadership as appropriate. Triage client and caregiver concerns when nursing input is needed. Provide clinical guidance and support to CNAs within the scope of non-skilled home care services. Monitor documentation and care delivery for consistency with established care plans. Collaborate with administrative and operations teams to promote quality, safety, continuity of care, and regulatory compliance.

Maintain timely, accurate, and professional clinical documentation.

QUALIFICATIONS

Current, unrestricted Registered Nurse (RN) license. Experience caring for individuals with Alzheimer's disease and other forms of dementia required. Home care experience preferred. Recent bedside nursing experience preferred. Strong assessment, critical-thinking, communication, and clinical documentation skills. Comfortable working independently and making sound clinical decisions within the RN's scope of practice. Highly organized, dependable, motivated, and able to effectively manage a community-based caseload. Strong interpersonal skills and the ability to communicate effectively with clients, families, CNAs, and members of the management team. Proficiency with Microsoft Word, Excel, and Outlook. Experience and proficiency with Electronic Visit Verification (EVV) and/or home care software. Reliable transportation and ability to travel to client homes as required.

SCHEDULE 24

hours per week. This is a part-time position involving a combination of client home visits, clinical documentation, care coordination, and communication with the agency team.

COMPENSATION

Competitive and negotiable based on experience, qualifications, and relevant home care/clinical background.
