•Male, high school senior. Significant needs classroom. Diabetes, Down Syndrome, Cerebral Palsy, wheel chair bond, full support for ADLs feeding, lifting for toileting. 2 person lift at this time.

Location:

Onsite in Middleton Responsibilities Provide 1:1 nursing support to a high school senior in a significant needs classroom Manage diabetes care and other health needs throughout the school day Support ADLs including feeding, toileting, transfers, and personal care Assist with wheelchair mobility and 2-person lifting as needed Provide academic and classroom support, as well as community vocational exploration Coordinate with parents, school staff, and healthcare providers as needed Maintain documentation and comply with school health and safety regulations Participate in IEP or 504 Plan meetings when relevant Qualifications Current and Unrestricted Rhode Island RN License 1+ years of pediatric nursing experience Experience supporting students with complex medical and developmental needs preferred Experience with diabetes, Down Syndrome, Cerebral Palsy, and wheelchair support a plus CPR and First Aid certification required Strong organizational, communication, and decision-making skills Compassionate, patient, and student-centered Why Join Sage Connections?
