3007327500-2 Apply for this job now Job Description HCA Healthcare is seeking an RN Cardiac Rehab to deliver high-quality, patient-centered care for individuals recovering from cardiac events and procedures. In this role, you will assess patients, develop individualized rehab plans, monitor vital signs and cardiac rhythms, and provide education on lifestyle modification and medication adherence. You will collaborate with cardiologists, therapists, and multidisciplinary teams to optimize outcomes. Responsibilities Assess cardiac rehab patients and develop individualized treatment and exercise plans Continuously monitor cardiac rhythms, vital signs, and patient responses to exercise Provide education on heart-healthy lifestyle, medications, and risk-factor reduction Document assessments, interventions, and outcomes accurately in the EHRCollaborate with cardiologists and interdisciplinary teams to optimize patient care Recognize and respond to cardiac emergencies following ACLS protocols Coordinate care transitions and follow-up plans for program participants Participate in quality improvement and patient safety initiatives within cardiac rehab Required Skills Cardiac rehabilitation nursing ECG/EKG monitoring and interpretation Cardiac risk assessment Exercise tolerance testing and monitoring Vital signs assessment and documentation Patient and family cardiac education Care planning and care coordination Telemetry monitoring systems Emergency cardiac response (ACLS-level) Electronic Health Record (EHR) documentation
