Key Responsibilities:

Deliver high-quality, in-home nursing care and skilled clinical interventions Conduct patient assessments and accurately complete OASIS documentation in a timely manner Collaborate closely with physicians and interdisciplinary care teams to ensure individualized care plans Provide wound care, medication management, patient education, and chronic disease monitoring Communicate patient status updates and condition changes to the clinical team promptly

Qualifications:

Active RN license Minimum of 1 year of recent RN experience in home health Demonstrated experience with OASIS (required) Proficiency in HomeCare HomeBase (strongly preferred) Ability to efficiently manage a full-time caseload with daily visits Reliable transportation and valid drivers license Whats in It for You?
