This role involves engaging with Medicare Advantage plan members throughout Dillon County, assessing both annual health and post-discharge needs. Each home visit typically takes about 45 minutes and candidates may be scheduled in locations such as Dillon, Latta, Lake View, and surrounding communities.

Key Responsibilities:

Complete annual home health risk assessments for Medicare Advantage plan members Review and evaluate medical and medication history, as well as current medical conditions Educate patients on their diagnoses and prescribed medications Identify gaps in care and recommend actions to close those gaps No responsibility for developing treatment plans or prescribing medications

Qualifications:

Active Nurse Practitioner license in South Carolina Current CPR certification (BLS) Board certification through AANP or ANCC At least 6 months recent nurse practitioner experience (preferred) MSN degree from an accredited nursing program In this rewarding role, you will receive $80-$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
