Key Responsibilities:

Perform comprehensive home health risk assessments (45-minute visits) for Medicare Advantage plan members Review and evaluate prior and current medical histories, chronic conditions, and current medications Educate patients on health conditions and prescribed medications, supporting their understanding and wellness Identify any gaps in care, providing actionable recommendations Conduct annual health assessments and post-discharge visits as needed No requirement for prescribing medications or developing new treatment plans

Qualifications:

Active and unrestricted Nurse Practitioner license in South Carolina Board Certification through AANP or ANCC Current BLS/CPR certification MSN degree from an accredited nursing program Minimum 6 months of current Nurse Practitioner experience preferred, but not required This role offers $80$90 per evaluation, along with a mileage stipend and compensation for administrative time, ensuring your expertise and travel are valued and supported.
