Located in Inman, SC, RoseCrest is a growing, robust community of active senior adults. Overview We are seeking a dedicated and detail-oriented MDS Case Manager to join our dynamic healthcare team. In this vital role, you will coordinate patient assessments, ensure compliance with federal and state regulations and facilitate seamless communication among interdisciplinary teams. Your expertise will help optimize patient care, support regulatory adherence and improve clinical documentation standards across diverse healthcare settings. Duties Conduct comprehensive MDS assessments for long-term care residents, ensuring accuracy and completeness in accordance with CMS (Centers for Medicare and Medicaid Services) guidelines. Review medical records, clinical documentation and treatment plans to verify compliance with healthcare regulations such as
HIPAA, HEDIS, NCQA
standards and state-specific long-term care policies. Collaborate with nursing staff, physicians, therapists, and other healthcare professionals to gather necessary information for accurate MDS coding and documentation review. Utilize EMR (Electronic Medical Records) systems like PCC to document assessments, update patient records and ensure timely submission of required data. Manage utilization review processes by evaluating patient care plans against Medicare and Medicaid regulations to support appropriate reimbursement and discharge planning. Stay current on changes in CMS regulations, ICD-10/ICD-9/ICD coding standards, CPT coding procedures, DRG (Diagnosis-Related Group) classifications and long-term care regulations to maintain compliance. Participate in quality improvement initiatives related to clinical documentation improvement (CDI), HEDIS metrics and health information management practices. Skills Extensive knowledge of PCC and proficiency in Microsoft Office tools for documentation and reporting purposes. Strong understanding of managed care principles, Medicare regulations. Critical care experience combined with nursing or medical office background enhances ability to perform thorough assessments. Experience with discharge planning processes for hospice care or emergency medicine settings is advantageous. Knowledge of health regulation policies such as
HIPAA, CMS
guidelines, state healthcare regulations related to long-term care and inpatient/outpatient services. Excellent organizational skills with the ability to review medical records meticulously while adhering to clinical documentation standards. Strong communication skills for effective collaboration across multidisciplinary teams involved in patient care management.
