Key Responsibilities:

Conduct comprehensive home health risk assessments for Medicare Advantage members Review and evaluate patients prior and current medical history, conditions, and medications Offer patient education regarding specific medical conditions and prescribed medications Identify gaps in care and provide recommendations for addressing them Visits focus on assessment and education; no prescribing or treatment plan development required

Qualifications:

Active Nurse Practitioner license in South Carolina Current CPR (BLS) certification Board Certification via AANP or ANCC Minimum of 6 months recent Nurse Practitioner experience preferred MSN from an accredited nursing program Enjoy a compensation structure that includes $80-$90 per completed evaluation, alongside a mileage stipend and reimbursement for administrative time.
