Key Responsibilities:

Conduct comprehensive annual health risk assessments during 45-minute home visits Review patients prior and current medical histories, health conditions, and medication regimens Deliver patient education regarding health conditions and medications to support understanding and wellness Identify gaps in care and offer actionable recommendations to close them No treatment plan development or medication prescribing required

Qualifications:

Active Nurse Practitioner license valid in South Carolina Board Certification through AANP or ANCC Current BLS/CPR certification MSN degree from an accredited nursing school At least 6 months of experience as a nurse practitioner (preferred)
