Job Description

As an RN Clinical Support Coordinator at HCA Healthcare, you will leverage your nursing expertise to coordinate patient care, support clinical teams, and optimize care pathways. In this role, you will perform telephonic and electronic triage, review clinical information, support scheduling and referrals, and ensure accurate EHR documentation. You will collaborate with physicians, nurses, and allied health professionals to enhance patient outcomes and access to services. Responsibilities Provide telephonic and electronic clinical triage, routing patients to appropriate level of care. Coordinate patient care plans, referrals, and follow-up appointments across multiple service lines. Review and document clinical information accurately within the EHR to support safe, timely care. Serve as a central point of contact for providers, staff, and patients to resolve care coordination issues. Monitor patients' clinical needs and escalate concerns to providers using established protocols. Support quality, safety, and utilization initiatives through data collection and adherence to guidelines. Educate patients and families on care plans, discharge instructions, and available resources. Collaborate with interdisciplinary teams to streamline workflows and improve patient experience. Ensure compliance with regulatory standards, policies, and confidentiality requirements. Participate in continuous learning, orientation of new staff, and process improvement projects.
