Family provides hospice services to patients in their place of residence in both Georgia and South Carolina. As the Registered Nurse Case Manager, you will the primary source of care management. You will work closely with the interdisciplinary team to ensure all patients receive individualized care on their end of life journey. Qualifications An enthusiasm for providing the best possible care to patients and their families. A Registered Nurse license in good standing 1 year of experience in hospice or home health preferred A valid driver's license Current CPR certification Excellent observation, verbal, and written communication skills
