Job Summary We are seeking a detail-oriented and clinically skilled Admissions Nurse (Registered Nurse) to join our clinical team. In this role, you will serve as the primary point of contact for new residents transitioning into our skilled nursing facility (SNF). You will be responsible for reviewing referrals, managing clinical intake logistics, executing initial clinical assessments, and managing employee health protocols. This role bridges clinical expertise with corporate compliance, ensuring that all admissions meet regulatory standards while maintaining an empathetic approach toward incoming residents and their families. This position offers the opportunity for a highly flexible schedule, allowing for adaptable work hours as long as the admission needs of the facility are successfully met. Duties & Responsibilities Referral Review & Intake Coordination Evaluate resident referrals from hospitals, managed care organizations, and community sources to ensure medical eligibility. Verify facility capability to meet the specialized clinical needs of prospective residents. Initiate the admission process dynamically by coordinating with the nursing, social services, and financial departments. Clinical Admission & Assessment Perform initial admission assessments, including a comprehensive head-to-toe evaluation, baseline vitals, skin checks, and cognitive screening. Review and document pre-existing health history and reconcile current medication profiles. Initiate physician admission orders to establish immediate care protocols, specialized therapy parameters, and dietary paths. Complete all mandatory intake paperwork accurately and timely according to state regulations and Medicare guidelines. Rotate RN supervisory role for the facility one weekend every 6-7 weeks. Assist with audits and related duties as assigned. Duties & Responsibilities continued… Insurance & Regulatory Compliance Assist with insurance update paperwork and provide necessary clinical documentation to managed care providers to secure authorizations. Liaise with case managers to ensure seamless coverage transitions and mitigate service coverage disruptions. Employee Health Services Conduct employee health assessments for new hires and recurring staff compliance checks. Administer and log required vaccinations, screenings (e.g., TB testing), and post-exposure protocol follow-ups.
Qualifications & Requirements Licensure:
Must be a Registered Nurse (RN) with an active, unrestricted license in the practicing state.
Education:
Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
Experience:
Minimum of 2 years of nursing experience, preferably within a skilled nursing, rehabilitation, or long-term care environment.
Technical Skills:
Proficiency with Electronic Health Record (EHR) systems and digital admission software.
Competencies and physical requirements:
Strong clinical acumen, rapid diagnostic critical thinking, excellent organizational habits, must be able to walk or stand for extended periods of time, prolonged periods of sitting at a desk and working on a computer, must be able to lift up to 50 pounds, and exceptional communication capabilities.
