Job Summary:
The Director of Social Services will assist the Administrator in implementing and overseeing a comprehensive social services program that provides support services for patients and families in the facility while demonstrating teamwork characteristics by building a positive team approach.
Duties/Responsibilities:
1. Plan, develop, organize, implement, evaluate, and direct a comprehensive social service program that offers support services for residents and their families. 2. Overseeing social services to coordinate and implement team conferences for residents and creating a forum for ongoing discussions between residents, their families/responsible parties, and the treatment team. 3. Supervising social services to coordinate discharge planning and assisting in the creation of an organized discharge plan for all residents. 4. Assist in the development, administering, and coordinating of department policies and procedures. 5. Ability to assist in the thorough process of all Medicaid applications. 6. Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Administrator. 7. Review department policies and procedures, at least annually, and participate in making recommended changes. 8. Participating in the resident council and listening to the concerns of the residents. 9. Develop and implement policies and procedures for the identification of medically related social and emotional needs of the residents. 10. Participating in community planning related to the interests of the facility and the services and needs of the residents and family. 11. Interview resident/families as necessary and in a private setting. 12. Plan memorial services quarterly. 13. Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Administrator as required. 14. Involve the resident/family in planning social service programs when possible. 15. Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident. 16. Provide information to residents/families as to Medicare/Medicaid, and other financial assistance programs available to the residents. 17. Provide consultation to members of our staff, community agencies, etc., in efforts to solve the needs and problems of the resident through the development of social service programs. 18. Participate in Medicare, pharmacy, and referral meetings. 19. Familiarization with NOMNC and advising patients of coverage end dates. 20. Assist the Quality Assessment & Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies. 21. Participate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directed. 22. Interview residents, or family members, as necessary, to obtain social history. 23. Adheres to all HIPAA guidelines. 24. Performs other related duties as assigned.

Education and Experience:
Must possess, as a minimum, an LBSW, state issued credentials for individuals who hold a Bachelor of Social Work from a CSWE- accredited program and have passed the ASWB Bachelors exam. Must be registered as a Licensed Baccalaureate Social Worker. Three to five years of health care experience is preferred.
Physical Requirements:
Prolonged periods sitting at a desk and working on a computer. Must be able to lift up to 15 pounds at times.
