Monday Friday, 8:00 AM 5:00

PM Responsibilities:

Complete thorough OASIS assessments and develop individualized care plans. Deliver skilled nursing services, such as wound care, IV therapy, chronic disease management, and medication administration. Monitor patient status, document findings, and communicate changes to the multidisciplinary care team and attending physicians. Educate patients and their families on medications, treatment plans, disease prevention, and safe care techniques. Maintain accurate, timely records using Home Care Home Base (HCHB). Collaborate closely with therapists, medical social workers, and home health aides to support seamless care transitions. Participate in regular case conferences, staff meetings, and professional development sessions. Ensure compliance with all regulatory and clinical standards in documentation and care delivery.

Qualifications:

Active RN license in the state of practice, in good standing. At least one year of recent field-based Home Health experience. Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools. Reliable transportation, valid drivers license, and up-to-date auto insurance.
