Key Responsibilities:

Conduct annual home health risk assessments for Medicare Advantage plan members during in-home visits, lasting approximately 45 minutes each Assess prior and current medical history, medical conditions, and medication use Deliver patient education regarding medical conditions and prescribed medications Identify gaps in patient care and make recommendations to support overall health Focus exclusively on assessmentsno care plan creation or medication prescribing required

Qualifications:

Valid Nurse Practitioner license in Tennessee Current CPR certification (BLS) Board Certification from AANP or ANCC At least 6 months of recent nurse practitioner experience preferred Master of Science in Nursing (MSN) from an accredited institution Nurse Practitioners in this per diem role receive $80$90 per evaluation completed, with additional compensation for mileage and administrative duties.
