Job Description

Signature HealthCARE seeks a compassionate Registered Nurse to deliver person-centered care in our long-term and post-acute facility. The RN will assess residents, develop and manage care plans, administer medications and treatments, and collaborate with an interdisciplinary team to support rehabilitation and quality of life. Responsibilities Provide person-centered nursing care to long-term and post-acute residents Perform assessments, develop and update individualized care plans Administer medications and treatments per provider orders and facility policy Monitor resident conditions, respond to changes, and notify providers and families Document care accurately in the electronic health record (EHR) Collaborate with an interdisciplinary team to support rehabilitation goals Educate residents and families on care plans, medications, and health management Support a safe, dignified, and compassionate environment aligned with mission and values Precept, mentor, and support team members to promote quality and teamwork Participate in quality improvement, regulatory compliance, and ongoing training Required Skills Patient assessment Care planning and documentation Medication administration Wound care management IV therapy Infection prevention and control Electronic health records (EHR) documentation Care coordination and discharge planning Clinical triage and prioritization Patient and family education
