In this per diem position, you'll independently conduct comprehensive evaluations for Medicare Advantage members residing in Hays County. You'll play a crucial role in fostering preventative care and closing gaps in treatmentall with the flexibility of self-scheduling and manageable visit durations.

Key Responsibilities:

Perform thorough annual home health risk assessments in members' homes Review each patients prior/current medical history, conditions, and medications Educate members on their medical profile and encourage adherence to prescribed therapies Detect gaps in care, making actionable recommendations to enhance patient outcomes Visits may involve annual assessments or post-discharge follow-up; no need to develop treatment plans or prescribe medications

Qualifications:

Active Nurse Practitioner License in Texas Board Certified via AANP or ANCC Master of Science in Nursing (MSN) from an accredited institution Current CPR certification (BLS) Minimum of 6 months experience as an NP preferred
