Key Responsibilities:

Perform thorough home health risk assessments for Medicare Advantage members Review and evaluate patients current and previous medical history, including medical conditions and medication usage Educate patients on their medical conditions and prescribed medications Identify care gaps and make informed recommendations for follow-up No requirement to create treatment plans or prescribe medications, focusing exclusively on assessment and education

Qualifications:

Active Nurse Practitioner license in Texas Current CPR certification (BLS) Board certification from AANP or

ANCC MSN
