This role involves delivering personalized patient care, managing complex medical needs, and collaborating with interdisciplinary teams. The ideal candidate will have extensive clinical experience across various specialties, including critical care, geriatrics, pediatrics, and chronic disease management. Familiarity with electronic health record (EHR) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks is preferred. The Home Health Nurse will play a vital role in promoting health, recovery, and well-being for diverse patient populations, including those with developmental disabilities, memory care needs, or requiring hospice and palliative services. Responsibilities Conduct comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations. Administer medications via injections, IV infusions, and catheter care while adhering to aseptic techniques and HIPAA regulations. Manage complex medical procedures such as wound suturing, ostomy care, ventilator management, and tube feeding. Perform diagnostic evaluations including laboratory specimen collection/processing and basic diagnostic assessments using EMR/EHR systems. Provide specialized care for patients with acute pain, infectious diseases, trauma, or requiring critical care interventions such as ICU or PICU experience. Develop individualized discharge plans and coordinate post-acute care or long-term community services. Educate patients and families on health coaching, medication administration, disease management (e.g., diabetes, cardiac conditions), and safety protocols. Collaborate with physicians and interdisciplinary teams to ensure continuity of care across settings like hospitals, nursing homes, assisted living facilities, and outpatient clinics. Maintain accurate medical documentation including CPT/ICD coding and ensure compliance with all healthcare standards such as NCQA standards. Skills Extensive clinical expertise across multiple specialties including emergency medicine, internal medicine, geriatrics, pediatrics, behavioral health, and hospice & palliative medicine. Proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks for documentation and patient tracking. Strong knowledge of medical terminology, anatomy & physiology, diagnostic evaluation techniques (sonography), and infection control practices. Ability to perform advanced procedures like spinal taps, gastrostomy tube management, ventilator support, ostomy care, and sterile processing. Experience working with diverse populations including children (neonatal to Toddler), elderly patients in memory or dementia care settings; individuals with disabilities; or those requiring specialized services like dialysis or occupational health. Skilled in patient assessment techniques including vital signs monitoring; airway management; venipuncture; phlebotomy; medication administration; wound suturing; CPT coding; ICD coding; and use of Hoyer lifts or other mobility aids. Strong communication skills for teaching patients/families about health management plans while maintaining confidentiality per HIPAA standards. Ability to work independently in home environments while adhering to safety protocols for infection control and aseptic technique.
