Job Description

Aveanna Healthcare is seeking a compassionate Private Duty LVN to provide one-on-one skilled nursing care to pediatric and adult patients in their homes. In this role, you will deliver clinically focused care, including assessments, medication administration, ventilator and tracheostomy management, and wound care, while closely monitoring and documenting patient status. You will collaborate with families and the interdisciplinary team to follow individualized care plans, educate caregivers, and support patient comfort, safety, and independence. Responsibilities Provide one-on-one skilled nursing care to pediatric and adult patients in the home setting Administer medications, treatments, and interventions per physician orders and care plans Monitor, assess, and document patient status, vital signs, and responses to care Manage ventilators, tracheostomies, feeding tubes, and other medical equipment as needed Collaborate with families, caregivers, and the clinical team to support patient goals Educate patients and families on disease processes, medications, and home care techniques Ensure compliance with infection control, safety, and clinical quality standards Participate in care plan development, updates, and interdisciplinary communication Maintain accurate, timely clinical documentation in electronic systems Support patient comfort, dignity, and independence while working independently in the field Required Skills Skilled nursing assessments Medication administration Ventilator and tracheostomy management Wound care Enteral feeding and tube management Electronic health record documentation Infection control practices Patient and family education Care planning and coordination Time management and autonomous field practice
