Job Summary Has principal responsibility for the total nursing care, assessment, planning, implementation, and evaluation of each assigned patient. Plans, provides, and documents discharge planning, teaching, and educates patients, family, and others to attain optimal health and continuity of care. Assures that the plan of care is carried out in a safe environment through performance and supervision of subordinate personnel. Assures quality nursing care of patients and fills education, consultant, and liaison rotes within nursing and for other nurses and for other departments. Job Qualifications Education Graduate of an accredited School of Professional Nursing. Personal Job-Related Skills Demonstrated ability in nursing practice. Leadership and management abilities. Effective oral and written communication skills. Computer skills/knowledge. Licensure, Registry or Certification Registered Nurse currently licensed by the State of Texas. BCLS required and ACLS preferred. Current CPR Certification. Prior Work Experience One year experience as a professional Registered Nurse preferred. One year in a nursing managerial or supervisory role preferred. Physical and Sensory Requirements (With or without the aid of mechanical devices) Position requires intermittent walking, sitting, and standing. Some bending stooping and reaching required with the lifting of items up to 50 pounds. Occasional heavy lifting when moving or supporting patients who may suddenly become weak or helpless. Must be able to move intermittently throughout the workday. Must be able to cope with the mental and emotional stress of the position. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met. Must function independently and have flexibility, personal integrity, and the ability to work effectively with patients, providers, personnel, and support agencies. May be necessary to assist in the evacuation of patients during emergency situations. Essential Duties and Responsibilities Utilizing the nursing process, completes a comprehensive physical and psychosocial assessment of patients. Takes nursing histories to provide a basis for developing patient care plans. Completes a thorough nursing assessment and history on admission. Identifies teaching/learning/discharge needs at time of admission and throughout hospitalization. Provides an ongoing assessment, which accurately reflects overt physiological and psychosocial needs of patients. Consistently uses input from patient, family, significant other, physician, and health care team member. Using nursing process, plan and provides patient/family care; evaluates patients' response to care provided and redirects care as necessary. Formulates appropriate nursing diagnosis and identifies patient problems from admission database and ongoing assessment. Establishes patient outcome goals that are realistic, specific, measurable, and progressive. Develops and implements nursing orders, which are specific and measurable. Initiates interdisciplinary communications and incorporates recommendations. Consults with patient and family and others to formulate care plans that reflect concern for holistic patient needs. Assesses and evaluates patient-response/outcome and makes appropriate modifications on the care plan. Reviews and updates care plan on a timely basis to reflect patient's status throughout hospitalization. Implements and monitors appropriate nursing care to meet patient's needs. Identifies expected outcome for planned nursing intervention and ensures vital information is passed between shifts. Administers medications according to policy including IV blood, and blood components as prescribed by physicians. Knows actions, indications, contraindication, precautions, adverse reactions, dosage and route or administration on all drugs before administering to patient. Verifies medication administration accuracy and performs the required checks on the medication administration record. Gives exchange report to other health care personnel providing accurate and complete information on each patient at the change of shift, when transferring a patient to another hospital, and at intervals during the shift. Assesses patients at the beginning of shift for acuity of patient needs/problems and establishes priority in which work activities are consistently completed by the end of the shift. Performs basic nursing skills competently to include but not limited to: intake and output, vital signs, physical assessment, urine testing, specimens/cultures, dressing changes, neurological assessments, personal hygiene for patient, skin care, glucose monitoring, airway/suctioning, basic oxygen administration, nasogastric tubes, tube feedings, urinary catheterization, drains, enemas, and intravenous catheterization. Recognizes, evaluates, and intervenes appropriately in a life-threatening situation. Recognizes variance of diagnostic tests, vital signs and assessments that should be reported to the physician and notifies physician in a time frame appropriate to the variance. Functions proficiently in unit nursing modalities. Participates in the health care educational needs for the client, family, or significant other with regard to patient care needs, Disease prevention and health maintenance measures. Develops and implements an individualized teaching and discharge plan at admission and throughout hospitalization. Initiates and documents progressive patient, family, or significant other teaching on admission and throughout hospitalization. Act as liaison between patient, physician, family or significant other, and other health disciplines. Maintains accurate and timely records according to hospital policy and procedures. Documents ongoing assessments, within time frame determined by policy, which accurately reflects overt physiological and psychosocial needs of patient. Accurately documents on each shift the nursing patient care activities and the patient's response to nursing interventions and delegated medical management. Maintains a patient care plan that is current and reflects individual needs or preferences of patients. Documents teaching completed and patient, family, or significant other response in the patient record. Documents interdisciplinary recommendations for plan of care. Transcribes and/or cosigns physician's orders completely and accurately according to hospital policy and practice. Completes all appropriate transfer and discharge forms. Maintains the standards of patient care for each nursing unit. Makes rounds at least twice each shift to assess acuity's, patient and family satisfaction, and staff accomplishments toward meeting patient needs. Assures that physician orders are carried out promptly and accurately. Assists nursing staff members with problems. Consults with Medical Staff to coordinate nursing and medical care plans and acts as liaison between physician and patient care team when necessary. Facilitates timely and appropriate decisions on operational problems and personnel management. Facilitates favorable relationships within nursing and between nursing and medical staff, and other departments. Maintains communication with physician to assure mutual understanding of patient care goals and to resolve problems. Maintains communication and problem solving with other hospital departments to assure quality patient care. Maintains a connection between the patient care unit and the organization. Assists with the management of department functions and supervision of department personnel. Directs the implementation of the nursing process in clinical practices. Oversees checking of E.R. supplies, crash cart, emergency equipment, and oxygen resuscitation equipment at the beginning of each shift, unless ER RN is on duty. Assigns LVN's and Nursing assistants based on skill level and patient needs to appropriate nursing units. Assists with the development of department policies, procedures, and practices. Assists in training and developing employees and makes recommendations concerning discipline or termination compliance with hospital policy. Monitors the utilization of approved budget resources with special attention to assuring appropriate staffing and supply levels. Acts rapidly and effectively and assess leadership role for patient and other employees during any emergency situation. Plans assesses directs and participates in the delivery off emergency room nursing care. Participates in setting standards for patient care and nursing practice in emergency room nursing. Determines the number and level of personnel needed to provide quality patient care. Records, evaluates, and treats cardiac arrhythmias appropriate. Manages the patient with respiratory insufficiency and/or difficulty including assisting with intubations, basic interpretation of ABG results, proper suctioning techniques, assuring proper endotracheal tube placement, assembling appropriate O2 supplies/equipment, and having knowledge of the various O2, apparatuses. Consistently titrates drug therapies, recognizes interactions, obtains written hemodynamic parameters, and exercises judgement regarding frequency of recording all monitored valves. Cares for the patients with invasive lines and assists with insertions. Demonstrates ability to care for patient with a head injury to include: complete neurological assessment with documentation, medications, positioning, and notification of physician of neurological changes. Cares for patient requiring invasive procedures of thoracentesis, chest tube insertion, including use assembly of equipment, explanation of procedure to patient, and assisting physician with insertion. Manages care of patient requiring intravenous antithrombotic agents according to established procedures. Manages care pf patients with fracture including CSM checks below injury with documentation, proper body alignment, and assisting with casting. Manages patients with medical diagnosis (R/O MI, Diabetes, COPD, GI Bleed, etc.) including stating pathophysiology for specific diagnosis, documenting nursing interventions the ER form, stating signs and symptoms of complications for diagnosis and notifying physician of any changes in condition in a timely manner. Performs triage on multiple patients, evaluates, and prioritizes them according to hemodynamic instability, loss of limb, and treats appropriately. Interprets lab values including hemoglobin, hematocrit, platelet count, white count, electrolytes, and clotting times. Provides psychosocial care to the patients and family. Facilitates favorable relationships within the emergency room unit and with the medical staff, and other hospital departments and seeks input regarding nursing care. Maintains positives and effective relationships with other departments and the medical staff Consults with physician to resolve problems and to obtain and provide patient care input. Maintains open communication and problem-solving atmosphere with other departments to achieve quality patient care. Assists with the management of Nursing Department functions and supervises the Emergency Room unit employees. Assists with the development of departmental policies, procedures, and practices and monitors for compliance and consistency once established. Provides input and facilitates staff input into decisions affecting nursing practice. Assists in selecting, hiring, orienting, developing, evaluating, disciplining, and termination of unit employees in compliance with hospital policy. Participates in the preparation of the department budget and monitoring the utilization of approved budget resources with special attention to assuring appropriate staffing and supply levels. Monitors and controls the utilization of the unit's approved budgeted resources. Manages the processing of patients for emergency service. Supervises the duties of nurse aides who assist in the Emergency room. Assures that the admission process of patients is completed, and all information and consents are obtained. Establishes procedures to obtain insurance information, signatures, and forms required to bill third party payers. Completes or supervises the completion of emergency room registration documents. Completes or supervises assists in the preparation of outpatient records for day surgery and observation patients. Assures all patients seen are recorded in the registration record. Performs other duties as deemed necessary by the CNO (ACON in absence of CNO), or Administrator. Other Significant Requirements Maintains confidentiality while always working with confidential matters (per HIPPA guidelines). Provides service direct to patients while maintaining a positive patient relations atmosphere. Interacts with patients, family members, medical staff members and other department managers on a continuous basis. Follows aseptic procedures and procedures for using precautions when involved in direct patient contact or handling potentially infectious materials. Performs other duties as deemed necessary by department manager, CFO, and hospital CEO. Adheres to Clay County Memorial Hospital personnel policies.
