As we continue to grow, Hospice Services is looking for compassionate and dedicated professionals to join our team. 1. Registered Nurse (RN) Provide hospice patient admissions, discharges, and patient visits. Assess and monitor patients' needs and condition. Perform medication reconciliation. Complete required hospice documentation and paperwork. Coordinate patient care with the hospice care team. Must be competent and experienced in hospice nursing responsibilities. 2. Licensed Vocational Nurse (LVN) Provide compassionate hospice patient visits and care. Monitor patients and report changes in condition. Perform medication reconciliation. Assist with patient care according to the hospice plan of care. Complete required hospice documentation. Must be competent in hospice LVN responsibilities. 3. Social Worker (SW) Provide social work visits and support to hospice patients and families. Listen to patients and families and provide emotional and practical guidance. Help families understand available hospice resources and support services. Assist with obtaining hospice consents and completing required paperwork. Collaborate with the hospice care team to support patient and family needs. 4. Chaplain Provide spiritual and emotional support to hospice patients and their families. Visit patients and offer compassionate listening and spiritual guidance. Support patients and families during end-of-life care. Respect and support each patient's individual beliefs, values, and preferences. Work collaboratively with the hospice care team to meet patients' spiritual and emotional needs.
