Key Responsibilities:

Perform thorough annual home health risk assessments for Medicare Advantage members Travel to patient homes within Hays County, including San Marcos and surrounding communities such as Buda, Dripping Springs, and Wimberley Review and evaluate prior and current medical history, chronic conditions, and medication profiles Provide clear education to members about their medical conditions and prescribed medications Identify gaps in care and recommend appropriate next steps or follow-up actions Focus exclusively on assessmentno requirement to develop treatment plans or prescribe medications

Qualifications:
