We are seeking a knowledgeable and organized MDS Coordinator to oversee the Resident Assessment Instrument (RAI/MDS) process and ensure accurate, timely assessments and documentation in accordance with Medicare, Medicaid, OBRA, and other regulatory requirements.

• What You'll Do Coordinate and manage the facility's MDS/RAI process from admission through discharge. Ensure MDS assessments are accurate, complete, and submitted timely in accordance with CMS and regulatory requirements. Collaborate with nursing, rehabilitation, social services, physicians, and other members of the interdisciplinary team . Review clinical documentation to ensure resident conditions and services are accurately captured. Monitor Medicare/Managed Care residents and assist with continued eligibility and reimbursement requirements. Participate in Case Management, Level of Care, Triple Check , and other required meetings. Conduct MDS audits and follow up on late, missed, or inaccurate assessments. Serve as an MDS/RAI resource and educator for facility staff. Stay current with Medicare/Medicaid reimbursement regulations and changes to MDS/RAI requirements.

• What We're Looking For Current RN or LVN/LPN license in the state of employment. Strong clinical assessment, documentation, organization, and communication skills. Ability to manage multiple deadlines and coordinate effectively with an interdisciplinary team. Strong analytical and problem-solving skills. Proficiency with standard office software and MDS/RAI-related systems. Willingness to complete required MDS/RAI training and ongoing competency requirements.
