Key Responsibilities:

Conduct thorough annual home health risk assessments in members' homes; each evaluation typically lasts about 45 minutes Review and assess both prior and current medical history, chronic conditions, and medication use Educate patients on their health conditions and medication regimens to promote optimal wellness Detect gaps in care and offer guidance to help members and their healthcare teams close those gaps Focus on assessment and educationno need to prescribe medications or initiate treatment plans

Qualifications:
