Job Summary:

Primary function is to administer nursing care under the supervision of a registered nurse for clients of all ages in their place of residence. Coordinate care with the interdisciplinary group, patient/family, and referring agency.

Job Qualifications:

Graduate of an accredited school of vocational nursing. Current state license as a Licensed Vocational Nurse; State Driver's License Two years experience as a Licensed Vocational Nurse in a clinical care setting, hospice or home health preferred. Nursing skills as defined as generally accepted standards of practice. Strong communication and interpersonal skills. Sensitivity to issues of loss and grief; Reliable transportation. TB health screening

Environmental and Working Conditions:

Works in patient's home in various conditions; ability to work flexible schedule and travel locally. Some exposure to unpleasant weather; PRN on call.

Physical and Mental Effort:

Prolonged standing and walking. Ability to move up to 50 lbs. and move patients. Requires working under possible stressful conditions to meet deadlines, to identify patients needs, to make quick decisions, meet patient/family psychosocial needs. Requires hand-eye coordination and manual dexterity. Ability to utilize durable medical equipment in the home.

Essential Functions:

Promote hospice mission, goals and philosophy Demonstrate compliance with policies, procedures, and regulations defining scope of practice. Promote security and safety of PHI Participate in ongoing education as required Assess patient/caregiver needs and provide education as required. Provide services according to plan of care and report changes to the RN. Attend IDG meetings as requested. Document services and submit timely.
