2 Description American Traveler is hiring an RN Case Manager for a Hospice/Palliative Care position in Loudoun County, VA, requiring a minimum of 1 year of recent hospice experience and a valid VA or compact RN license. Details Community/home-based hospice setting operating out of a hub office in Loudoun County, VA Geriatric patient population requiring end-of-life care Caseload of approximately 5-7 patients per day Day shift schedule, 5x8-hour shifts (8:00 AM - 5:00 PM) No on-call responsibilities Territory covers up to a 60-mile radius from the hub office; flexibility with territory is expected

EMR:

Netsmart iPad provided for use in the field Requirements Active VA RN license or compact RN license currently in hand Current BLS certification required Minimum 1 year of recent hospice nursing experience required Strong time management skills with an emphasis on timely documentation and communication Additional Information Responsibilities include managing a caseload of hospice patients in a community/home-based setting across an assigned territory Scrubs required in any color; jeans are not permitted Mileage is reimbursed at the IRS standard rate Local candidates are welcome to apply Two professional references from a supervisor within the past 2 years are required for consideration All time-off requests must be disclosed upfront during the consideration process
