Job Overview We are seeking a compassionate and skilled Registered Nurse (RN) to join our home health team. The ideal candidate will possess a strong clinical background, excellent patient assessment skills, and a commitment to delivering exceptional patient-centered care. As a Registered Nurse Home Health, you will be responsible for providing skilled nursing services in patients' homes, developing care plans, and coordinating with healthcare teams to ensure optimal health outcomes. "Company car is provided". Responsibilities Conduct thorough patient assessments to determine individual needs and develop personalized care plans aligned with physicians' orders. Administer medications and treatments accurately, including injections, IV infusions, wound suturing, and catheter care. Monitor vital signs, perform physical examinations, and evaluate patient progress regularly. Educate patients and their families on disease management, medication administration, and health maintenance strategies. Collaborate with interdisciplinary teams to coordinate care across settings such as hospitals, nursing homes, assisted living facilities, and community clinics. Document all patient interactions meticulously using Electronic Health Record (EHR) systems like Epic or Cerner while adhering to HIPAA regulations. Manage complex cases including chronic conditions such as cardiac issues, respiratory problems requiring ventilator support, infectious diseases, or memory care needs. Experience Candidates should demonstrate prior experience in diverse clinical settings including hospital medicine, critical care units such as ICU or Level I Trauma Centers, outpatient clinics, or post-acute care environments. Experience working with individuals with disabilities or in specialized areas like hospice & palliative medicine, geriatrics, behavioral health, or pediatric care is highly valued. Familiarity with medical coding systems such as ICD-10/ICD-9 and CPT coding is preferred. The ideal applicant will have proficiency in aseptic techniques, laboratory procedures including specimen collection/processing, and advanced skills in medication administration—including injections and IV insertions. Prior experience with EMR/EHR systems such as Athenahealth or eClinicalWorks is advantageous. A valid RN license with current CPR certification (Basic Life Support) is required along with critical thinking skills necessary for triage situations and acute pain management scenarios. $45.00 - $50.00 per hour
