Job Description

Aveanna Healthcare is seeking a compassionate Private Duty Registered Nurse to deliver one-on-one skilled nursing care in the home. In this role, you will assess, plan, and implement individualized care plans, administer medications and treatments, monitor chronic and acute conditions, and provide patient and family education. You'll collaborate with physicians, therapists, and the Aveanna care team to ensure high-quality, patient-centered care. Responsibilities Provide one-on-one skilled nursing care in patients' homes according to physician orders and care plans Perform comprehensive patient assessments and update individualized plans of care Administer medications, treatments, and therapies safely and accurately Monitor and manage chronic and acute conditions, escalating changes in status promptly Document all care provided in the EMR in accordance with regulatory and Aveanna standards Educate patients and families on disease processes, medications, and home care techniques Collaborate with physicians, therapists, and the interdisciplinary team to coordinate services Maintain infection control practices and ensure a safe home care environment Support a compassionate, patient-centric culture aligned with Aveanna's values Participate in ongoing training, competencies, and quality improvement activities Required Skills Patient assessment Care planning and care coordination Medication administration and reconciliation IV therapy and infusion management Wound care management Ventilator and tracheostomy care Pediatric and adult chronic disease management Electronic medical record (EMR) documentation Patient and family education Infection control and safety protocols
