Deliver skilled and compassionate nursing care to patients in the comfort of their homes as a Home Health Registered Nurse. Schedule:

Monday Friday, 8:00 AM 5:00 PM Full-time, 13-week contract

Caseload:

2532 points per week (35 patient visits per day)

Key Responsibilities:

Perform comprehensive OASIS assessments and develop individualized patient care plans Provide skilled nursing services: wound care, medication administration, IV therapy, chronic disease management Monitor patient progress, identify and communicate status changes to the care team and physicians Educate patients and their families on treatment plans, medications, and prevention strategies Complete accurate, timely documentation using Home Care Home Base (HCHB) Coordinate care with therapists, social workers, and home health aides for optimal outcomes Participate in case conferences, staff meetings, and ongoing education opportunities Adhere to regulatory standards and best practices in documentation and patient care

Qualifications:

Active RN license, valid and in good standing Minimum 1 year of recent home health (field) experience Proficiency in Home Care Home Base (HCHB) and OASIS required Strong clinical assessment, organizational, and communication skills Reliable transportation, valid drivers license, and auto insurance Ability to work independently and effectively prioritize a flexible caseload BSN and Medicare/home health compliance experience preferred Benefits include weekly pay, medical/dental/vision insurance options, 401(k) retirement plans, 24/7 support, mileage reimbursement, and potential for contract extension.
