Responsibilities Provide direct nursing care to residents across levels of care, including assessments, care planning, interventions, and evaluations. Administer medications and treatments safely and accurately in accordance with physician orders and facility policies. Monitor and document resident conditions, promptly recognizing and reporting changes to physicians and the interdisciplinary team. Maintain complete, accurate, and timely electronic health records and required regulatory documentation. Lead and support CNAs and other care staff, promoting teamwork and adherence to care standards and infection-control protocols. Participate in care conferences, quality improvement initiatives, and performance improvement activities. Educate residents and families on diagnoses, medications, discharge plans, and health management strategies. Ensure compliance with federal, state, and facility regulations, including safety and infection-prevention guidelines. Collaborate closely with physicians, therapists, social workers, and other team members to coordinate holistic, person-centered care.
